The Role
I’m looking for an experienced Advanced Practitioner Social Worker to join an acute hospital team in Somerset. You’ll be working within an intermediate care model, supporting adults as they transition through hospital and community pathways.
This is a frontline role where your expertise will make a real difference in complex cases, helping to ensure safe, timely discharges and person-centred care planning.
