Role Overview:
We’re looking for a qualified Social Worker to join a supportive team on a temporary basis, covering sickness absence. You’ll work within adults’ social care, providing high-quality support and guidance to individuals in the community, promoting independence, wellbeing, and positive outcomes.
Key Responsibilities:
Manage a complex caseload, carrying out assessments, support planning, and reviews for individuals with complex needs, and coordinate outcomes that promote independence.
Undertake assessments using a variety of health and social care tools, including contributing to continuing healthcare assessments.
Identify and provide access to Reablement and enablement services where appropriate.
Promote a Strength-Based Approach, helping individuals assess their own needs and make decisions that support their short- and long-term goals, delaying the need for long-term care.
Identify and utilise community and natural support resources, maximising individual assets through preventative services and funding options.
Monitor and review service provision, ensuring coordination and a focus on wellbeing and positive outcomes.
Liaise with local providers and support networks to improve service delivery.
Provide professional, detailed advice and support to service users, carers, colleagues, and staff, building positive relationships and partnerships.
Undertake safeguarding responsibilities as a Safeguarding Officer.
Key Accountabilities:
Maintain professional social work standards and be accountable for your own performance.
Ensure high-quality work and raise any issues that could impact service quality, budgets, or performance.
Support managers in achieving service targets within allocated resources.
Help maintain effective partnerships and contribute to a safe, supportive working environment.
Take care of your own health, safety, and welfare, and that of others affected by your work.
Undertake roles such as Approved Mental Health Practitioner (AMHP), Best Interests Assessor (BIA), or Practice Educator (PE) when trained and approved.
Contribute to service development, mentoring, and supporting less experienced staff.
Organise and manage your workload independently while developing your skills under supervision.
Maintain confidentiality and handle sensitive information appropriately.
Candidate Requirements:
Qualified Social Worker with relevant experience in community social work or therapy.
Strong communication, organisational, and relationship-building skills.
Ability to work independently and as part of a team.
Commitment to promoting independence, wellbeing, and safeguarding principles.
