The Role
We’re looking for a skilled Adults Social Worker to join a busy Community Care team. This role focuses on supporting vulnerable adults and their carers to live as independently as possible, whilst ensuring their safety, wellbeing, and choices are at the centre of everything you do.
You’ll carry out assessments, develop care and support plans, manage risk, and work closely with partner agencies. This is a role where you can really make a difference to people’s day-to-day lives while maintaining professional standards and embracing a strengths-based approach.
Key Responsibilities
Undertake assessments and reviews with adults and carers, identifying needs and risks.
Develop and deliver care/support plans that promote independence and personal outcomes.
Work within safeguarding procedures, including Protection of Vulnerable Adults (POVA).
Manage a caseload effectively, balancing priorities and statutory requirements.
Collaborate with health, housing, voluntary sector and other agencies through multi-disciplinary working.
Advocate for citizens and carers to ensure their voices are heard and respected.
Maintain accurate records and provide clear reports.
Support unqualified team members through supervision and guidance.
What We’re Looking For
Qualified Social Worker (Diploma/Degree or equivalent).
Social Care Wales registration.
Enhanced DBS (Adult Barred List).
Solid knowledge of:
Social Services & Wellbeing (Wales) Act
Mental Health Act / Mental Capacity Act
Human Rights Act
Experience in adult services, care management, and multi-disciplinary working.
Strong communication and ICT skills (WCCIS desirable).
Ability to work independently, manage risk and deliver positive outcomes.
Full UK driving licence and access to a vehicle.
