Mental Health Practitioner Band 6 Contract

Working Hours: Monday to Friday, 9:00 AM – 5:00 PM

Are you a dedicated and compassionate Mental Health Practitioner looking to make a real impact? We are seeking a Band 6 Mental Health Practitioner to join our team in Chorley. This role offers the opportunity to provide holistic care and support to service users throughout their recovery journey.

Key Responsibilities:

• Conduct comprehensive assessments of needs and deliver care from assessment to discharge.
• Develop, implement, and evaluate individualised care plans collaboratively with service users, carers, and other professionals.
• Empower and support service users on their recovery journey, ensuring person-centred care.
• Work collaboratively across professional and organisational boundaries to deliver holistic assessments.
• Oversee and manage clinical risk through robust risk assessments and mitigation plans.
• Advocate for individuals using the service, ensuring their needs are met effectively.
• Provide supervision and support for non-qualified staff.
• Demonstrate a basic understanding of the Mental Health Act.
• Collaborate with external partners, including community-based services, social services, and healthcare agencies.
• Deliver therapeutic interventions as required and provide signposting to additional services.
• Take responsibility for conducting new patient assessments within the team.
• Build and strengthen mental health knowledge and links between community teams.

Pay Rates:
Days: £22 – £26 per hour
Nights/Saturdays: £30 – £34 per hour
Sundays/Bank Holidays: £38 – £44 per hour

Requirements:
Proven experience in mental health care, particularly within community settings.
Strong skills in risk assessment, care planning, and providing therapeutic interventions.
Ability to work collaboratively with multidisciplinary teams and external agencies.
Commitment to delivering high-quality, person-centred care.
