Band 5 Community Staff Nurse – Cirencester
📍 Location: Integrated Community Teams, Cirencester
🕒 Hours: Full-time (hours negotiable per rota)
⚡ Urgent Requirement – Immediate start
Gloucestershire Health and Care are urgently seeking Band 5 Community Nurses to join their Integrated Community Teams in Cirencester. This role is ideal for skilled nurses with community experience who are confident delivering care in patients’ homes and clinic settings.
Key Responsibilities
As a Band 5 Community Nurse, you will:
- Deliver high-quality nursing care to patients in their own homes or community clinics
- Conduct holistic assessments, create and evaluate individualised care plans
- Provide wound care, catheter care, and administer medications, including intramuscular and subcutaneous injections
- Maintain accurate patient records using electronic systems (System One)
- Act as Named Nurse when appropriate, providing oversight for a defined caseload
- Participate in clinical triage, referrals, and prioritisation of workload
- Collaborate with multidisciplinary teams, social care, primary care, and external agencies
- Support students and junior staff through teaching, mentoring, and supervision
- Contribute to audits, quality initiatives, and service development projects
- Travel independently to patient homes (car required)
Candidate Requirements
Essential:
- Registered Nurse (NMC) with degree or diploma in nursing
- Demonstrable community nursing experience
- Skilled in wound care, catheter management, and medication administration (IM & SC injections)
- Competent in System One electronic record keeping
- Own transport and full driving licence
- Strong communication, organisational, and prioritisation skills
- Able to work independently and under pressure
Desirable:
- Experience mentoring or supervising student nurses
- Additional post-registration training or qualifications in community care
- Teaching & assessing experience
Pay Rates (Band 5 – NHSI Cap Rates)
Day: £21.00 per hour
Night / Saturday: £27.50 per hour
Sunday / Bank Holiday: £33.00 per hour
