The Role
We’re looking for an experienced Children’s Social Worker to join a specialist Child to Adult Transition Team. This team supports children and young people with disabilities and their families, ensuring a smooth transition into adult services.
This is a frontline role where you’ll be working across both children’s and adults’ legislation — providing continuity of support and high-quality social work practice throughout each young person’s journey. You’ll be responsible for managing your own caseload and delivering meaningful interventions that make a difference.
Key Responsibilities
Build and maintain strong, supportive relationships with children, young people and their families.
Assess needs, identify risks, and take appropriate safeguarding action where necessary.
Lead and contribute to care planning, case conferences and multi-agency meetings.
Deliver practical and emotional support that promotes development and independence.
Manage child protection cases, including preparing reports and giving evidence in court if required.
Ensure records, assessments and plans are completed accurately and on time.
Work collaboratively with partner agencies across health, education, and social care.
Champion equality, challenge discrimination, and advocate for service users.
What We’re Looking For
Qualified Social Worker (degree or equivalent).
Current Social Care Wales registration.
Enhanced DBS required.
Strong working knowledge of:
Children Act 1989/2004
Social Services & Wellbeing Act (2014)
Mental Health & Capacity Acts
Human Rights Act
Child Protection & Adult Care procedures
Proven experience working with children and families, ideally within disability or transition services.
Excellent communication and organisational skills.
Confidence in managing complex caseloads, safeguarding work, and multi-agency collaboration.
