Salary: USD $211,328.00 to $333,673.00

The Role

The Director of Care Management will provide strategic and operational leadership for all care management functions, including case management, utilization management, discharge planning, and psychosocial interventions. This leader will be accountable for patient outcomes, satisfaction, and care continuum utilization while ensuring services align with best practices.

What You’ll Do

• Care Management & Transition Leadership: Oversee care transitions, utilization management, and discharge planning, ensuring safe and patient-centered processes.
• Operational Excellence & Performance Improvement: Develop strategic work plans and monitor departmental performance metrics to drive improvements.
• People Leadership & Workforce Development: Supervise and develop teams, providing ongoing education and professional development opportunities.
• Physician, Executive & Community Collaboration: Build relationships with stakeholders to enhance patient experience and care coordination.
• Financial Stewardship & Accountability: Manage budgets, staffing plans, and operational metrics while ensuring quality outcomes.
• Governance, Compliance & Reporting: Ensure compliance with regulatory standards and lead reporting efforts related to care management outcomes.

What You’ll Need

• Education: Bachelor’s degree in nursing and a master’s degree in a health-related/science field.
• Experience: Minimum of 2 years in care management within an acute-care setting and 2 years in healthcare leadership related to acute-care case management.
• Licenses & Certifications: Active RN license and a California Fire and Life Safety Card (required within 30 days of hire).
• Skills: Strong knowledge of acute-care case management principles, excellent problem-solving and communication skills, and proficiency in Microsoft Office applications.
