Job Title: Band 6 CHC Practitioner
Organisation: NHS Bath and North East Somerset, Swindon & Wiltshire
Location: Remote (occasional face-to-face assessments may be required)
Contract: Temporary (until 31 December 2025)
Hours: Monday–Friday, 9am–5pm (some flexibility may be required)
Pay: £21 per hour PAYE / £26 per hour Umbrella
The Role:
We are looking for experienced CHC Practitioners to join the NHS Bath and North East Somerset, Swindon & Wiltshire team, carrying out Continuing Healthcare (CHC) assessments and reviews. These roles are predominantly remote, with occasional in-person assessments where required.
Key Responsibilities:
Conduct CHC assessments and reviews in line with statutory guidance
Prepare and maintain accurate documentation
Communicate effectively with patients, families, and multi-disciplinary teams
Contribute to service improvement initiatives and best practice
Requirements:
Experience in CHC assessments and review processes
Excellent communication and organisational skills
Ability to work independently and manage caseloads remotely
