Job Title: Band 6 PHB Practitioner
Organisation: NHS Bath and North East Somerset, Swindon & Wiltshire
Location: Remote (occasional face-to-face assessments may be required)
Contract: Temporary (until 31 December 2025)
Hours: Monday–Friday, 9am–5pm (flexibility may be required)
Pay: £21 per hour PAYE / £26 per hour Umbrella
The Role:
We are looking for an experienced PHB Practitioner to join NHS Bath and North East Somerset, Swindon & Wiltshire, supporting Personal Health Budget (PHB) planning and implementation. This role involves completing Care and Support Plans and ensuring PHB guidance is applied effectively.
Key Responsibilities:
Develop and maintain Care and Support Plans for individuals with PHBs
Apply PHB guidance in practice to meet individuals’ needs
Liaise with patients, families, and healthcare professionals
Maintain accurate records and contribute to service quality
Requirements:
Knowledge and experience in PHB processes and Care and Support Planning
Excellent organisational and communication skills
Ability to work independently and manage remote caseloads
