Rate: £24.65 per hour PAYE / £32.15 per hour Umbrella
The Role
We are seeking an experienced Social Worker to join our Adults Services team, supporting older people during a temporary peak in workload. This role involves assessing needs, delivering high-quality care, and working closely with colleagues and partners to ensure positive outcomes for service users.
You will manage a caseload, undertake assessments, develop care plans, and provide timely interventions while ensuring statutory responsibilities are met.
Key Responsibilities
Conduct assessments for older adults and their carers
Develop and implement care plans tailored to individual needs
Monitor, review, and adjust care packages as required
Work collaboratively with multi-disciplinary teams and external partners
Maintain accurate records and ensure compliance with statutory requirements
Support service improvement initiatives and best practice
About You
Fully qualified in Social Work and registered with Social Work England
Experience in adult social care, ideally with older people
Strong understanding of statutory frameworks and safeguarding requirements
Confident managing complex cases with autonomy
Excellent communication, planning, and organisational skills
Committed to providing high-quality, person-centred care
